From San Francisco
Take I-80 East across the Bay Bridge into Oakland. Continue onto I-580 East and take the 11th/12th Street exit. Turn right onto 12th Street; the campus is approximately three blocks east on the left, between Madison Street and Oak Street. Allow roughly 25 to 35 minutes from downtown San Francisco in standard traffic.
From the South Bay and Peninsula
Take I-880 North to the 7th Street exit, then turn right onto Madison Street and right again onto 12th Street. The facility is half a block east. Allow approximately 50 minutes from San Jose, longer in commute hours.
From the East Bay
From Berkeley or Albany, take CA-13 South or surface streets to downtown Oakland; turn east on 12th Street. From Hayward or Fremont, take I-880 North to the 7th Street exit and follow the South Bay directions. From Walnut Creek or the I-680 corridor, take CA-24 West to I-580 West, exit at 11th/12th Street.
BART access
The 12th Street/Oakland City Center BART station is one block from our front entrance. The station is served by all four East Bay lines.
Arrival notes
Limited parking is available on site for admissions arrivals; metered street parking is available throughout downtown Oakland. If you are driving someone in crisis, pull directly into the admissions semicircle and a nurse will meet the vehicle. The address is 225 12th St, Oakland, CA 94607.
HHPL Rehab primarily serves patients across Oakland, the broader East Bay, and the Greater Bay Area - Alameda, Berkeley, Albany, Emeryville, San Leandro, Hayward, Fremont, Castro Valley, Walnut Creek, Concord, Richmond, El Cerrito, and the surrounding communities, with regular admissions from San Francisco, Marin, and the Peninsula. Our 12th Street location was chosen deliberately for transit accessibility and proximity to the East Bay hospital network.
Hospital-to-hospital transfers are coordinated directly with referring clinicians at Highland Hospital, Alta Bates Summit, Kaiser Oakland, Children's Hospital Oakland, and the broader Sutter and Kaiser East Bay networks. Transfers typically complete within a few hours of the initial call.
Patients traveling from outside the Bay Area are welcome. Our admissions team coordinates pickup from OAK, SFO, and SJC airports and handles pre-admission logistics so the first day is about arrival, not navigation.
